Membership Enrollment

 

  •  Valid ID and Credit Card

  •  Credit card will be charged the at the time of your appointment.

  • Your first treatment of the month is included with the membership. Any additional treatments during the month are at membership prices

  • Club memberships are not refundable or transferable.

  • Membership discounts is only on regular priced items and services

  • Please provide 24 hours notice of cancellation

  • Not valid with any other Specials or Offers

  • Membership fees may not be paid with Gift Cards/Certificates

  • Your membership must be active in order to redeem any membership services

  • All membership cancellations require 30 days written notice and are effective 30 days after the date received. Payments due prior to the effective date will be charged as scheduled
